Information regarding Transcripts and Certification of Studies can be found in Section 7 of IPESA (Institutional Policy on the Evaluation of Student Achievement).
Official Transcripts
Official transcripts are normally required by an educational institution or an employer. Two official copies are sent directly to the institution with one student copy sent to the student at the same time as confirmation of the request. The cost is $5.00 per institution. To make a request, you must either come into the office and complete a form or make a written request including the student’s name and address, the name and address of the institution, your Champlain I.D. number. Requests will only be processed if they are accompanied by the $5.00 fee. You can download our Transcript Request Form in PDF format and fax it to 450-672-8297, or the form can be scanned and emailed to: records@champlaincollege.qc.ca. Every effort is made to process request for official transcripts within three days of receiving the request.
Student Copies of Transcripts
A student copy of a transcript for personal use may be obtained by coming into the office or by mail. The student’s name, Champlain I.D. number or date of birth is required. For confidentiality reasons, if you request someone else to pick up a student copy of your transcript, we require that they bring a letter of authorization from the student.
